Cremation Authorization
The authorization the legal next of kin signs before a cremation can take place. It names who is authorizing it and confirms the decisions made at the arrangement.
Authorization for Final Disposition
Wisconsin's state form (F-00086). It names who has the legal right to make the final arrangements, and it is also how a person names their own representative ahead of time. We post it exactly as the state issues it.
Release of Cremated Remains
The slip that names who may receive the cremated remains. Signed when the remains are picked up, so the right person walks out with them.

Veteran Memorial Engraving Request
For requesting engraving on a veteran's memorial marker. The fee for the engraving service is $106.00, listed right on the form.
