Evelyn M. Perry, age 97 of Eau Claire passed away peacefully on November 1, 2017.
Evelyn is survived by her daughter, Janice (Dale) Smith; son, Charles (Julie) Perry; and daughter, Nancy (Martin) Hendricks; 9 grandchildren; 15 great-grandchildren; and 7 great-great-grandchildren.
She was preceded in death by her husband, Dick Perry; daughter, Joan Perry; and great-grandson, Jacob Lindholm.
Private family internment will be held. In lieu of flowers, memorials may be sent to Augusta Health and Rehabilitation, 901 Bridge Creek Lane, Augusta, Wisconsin 54722.
